To Our Patients:
Laconia Clinic has always respected the confidentiality of your personal health information and medical treatment. Nothing is changing in this regard! You have a right to know that your information is protected by the Clinic, as spelled out in the Healthcare Insurance Portability and Accountability Act (HIPAA).
When you visit the Clinic, you will receive four forms that provide a description of your healthcare privacy rights. After you read these materials, you will keep one form and the other three need to be completed and signed for us to include with your Clinic records.
You will need extra time to read and understand the information and forms, so please arrive early for your appointment in order to avoid delays for you and your physician. If you have any questions, our staff is available to assist you. While we would prefer to have the forms signed on the day of your visit, you may take them home for more careful review and then return them at a later date, either upon your next visit or by mail.
Having signed forms in your medical record is essential to our providing care to you. In the future you may change whatever you have written on the forms by simply requesting new forms from our reception or nursing staff.
